Phase 1
Getting to Know You

If you have symptoms suggestive of POTS or hold a confirmed POTS diagnosis, we would love to help! We believe in providing holistic care by deeply understanding the challenges you face and the goals you hope to achieve.

Once you have signed up for an initial consultation appointment, you will receive a comprehensive intake form with key questions about your medical history and symptoms. The Health Vista team will review the form internally upon submission.
Phase 2
Consultation

The goal for consultation is to fine tune our understanding of your health journey and build a complete account of your symptoms and health timeline. We may ask for additional medical records to assist in our
understanding of your health.
​
At the end of the consultation, we will provide an assessment and plan. We will relay the recommendations back to your primary provider, and offer guidance regarding potential treatment, diagnostic testing, and referral to other specialists.
Phase 3
Continued Care

We believe caring for POTS necessitates a strong longitudinal relationship between the patient and the provider to achieve symptom improvements and a better quality of life.
We recommend follow-up visits to
reassess your symptoms and adjust your care plan as needed.
We will check in with you periodically, and empower you to do the same. We will be sure to keep the entire care team informed.
Billing & FAQ
How does Health Vista work?
Health Vista provides telehealth consultations to patients who have symptoms functionally suggestive of POTS or hold a confirmed POTS diagnosis. Health Vista is currently only available to Illinois and Colorado residents. We are hoping to expand to other states in the near future.

Do you take insurance?
Not at this time. However, we will generate a bill that you can submit to your insurance for reimbursement. Insurance companies may reimburse some, or even all, of the appointments. Please consult with your insurance company for policy details.

How do I pay?
Both the initial consultation and follow-up appointments are 1-hour sessions and we charge $500 for each appointment. ​We accept all major credit cards and ACH bank transfers.

What is your cancellation policy?
Patients may cancel or reschedule an appointment anytime 24 hours before the appointment. If you cancel with less than 24 hours, or fail to show up, you will be charged 100% of the consultation fee.​​​​​

Do you need education accommodations?
Health Vista can provide documentations to support a 504 plan and, in some cases, an Individualized Education Program (IEP). A 504 plan is a formal plan to provide accommodations and modifications for students with disabilities, ensuring they have equal access to education. An Individualized Education Plan (IEP) is a written plan outlining the special education services.
